If you are affiliated with an AFP and wish to continue paying this company only an amount deducted from your salary, the Superintendency of Banking, Insurance and AFP (SBS) reminds you that you have to complete a procedure.
1. In-person mode. With your ID, go to an AFP branch to which you contribute and request the 'Commission selection on remuneration' form.

2. Certified mail. Those who do not have a branch nearby must download the form from their AFP's website, fill it out, sign it and – with a legible copy of their ID – send it to the address indicated by their administrator.
In the case of Prima, for example, you must send them to the Commission Type Selection department, Calle Chinchón 980, San Isidro district, Lima. If you are in Horizonte, the address is Av. República de Panamá 3055, Floor 5 – Hall 2, San Isidro, Lima.
For Integra, the envelope must say 'POST BOX N° 27A008' and must be sent via Serpost.
If you are in Profuturo, the address is Andrés Reyes Street 489, San Isidro, Lima.
3. Website. Although the SBS regulations contemplate this option, no AFP has it in operation, but they assure that "they will do so soon."
If you do not complete these processes, you will be automatically migrated to the mixed commission, which has two components: a salary discount and a discount from the fund that you accumulate with your new contributions.
